Objective
PROMETHEUS addresses a major deficiency in prostate cancer management. Although localised disease achieves >95% 5-year survival, metastatic prostate cancer has a 5-year survival rate of 37%. Randomised trials show that PSA-based screening reduces mortality by 20-44%, yet uncoordinated screening yields up to 60% negative biopsies and ~30% indolent tumours, exposing men to avoidable procedures and contributing to annual prostate cancer-related costs of €5.8 billion in Europe. Current risk calculators reduce high-dimensional imaging to scores, obscuring tumour heterogeneity and leaving clinicians to interpret imaging, pathology and clinical records without an integrative decision-making framework.PROMETHEUS proposes a transformative shift from static scoring to dynamic, patient-specific disease-trajectory modelling. Its multimodal, multi-agent architecture integrates electronic health records, full-sequence MRI, PET/CT staging and long-term oncological outcomes. Specialised agents for detection, classification, data augmentation and decision reasoning learn joint cross-modal representations and are orchestrated as an AI super-agent for diagnosis and therapy decision support. A conversational clinical interface provides uncertainty-aware predictions and knowledge-based, explainable outputs under transparent clinician oversight. To improve privacy-preserving data sharing, PROMETHEUS develops a validated synthetic-data framework to generate realistic prostate MRI. By improving the prediction of aggressive prostate cancer and long-term outcomes through multimodal data integration, PROMETHEUS aims to reduce overdiagnosis and to personalise therapy decisions. Six European institutions (DKFZ, Erasmus MC, Heidelberg University Hospital, Karolinska Institutet, Radboud UMC and Umeå University) contribute cohorts exceeding 20,000 patients as well as deep expertise in prostate cancer, imaging and AI, enabling robust development, multi-site validation and clinical translation.
