Objective
Pancreatic cancer is a devastating disease with poor survival due to late diagnosis. Current imaging modalities fail to detect the disease early enough, even in high-risk individuals enrolled in structured surveillance programs. To improve outcomes, biomarkers must be developed that detect high-grade dysplasia, the final premalignant stage, before progression to incurable cancer. This ERC Advanced Grant proposal, PRECISE-PANC, aims to develop such biomarkers using a uniquely powerful biofluid: pancreatic juice.The applicant coordinates the world’s largest longitudinal biobank of pancreatic juice from individuals at high familial risk of pancreatic cancer (CAPS cohort). Furthermore through this proposal juice will be collected from the major European centres surveilling high risk individuals providing unprecedented potential to validate a novel biomarker. Drawing from this cohort, the project will apply advanced molecular profiling, specifically cfDNA fragmentomics and O-glycomics, to identify biomarkers with the sensitivity and specificity required for clinical decision-making. These technologies, not yet applied in combination to pancreatic juice, offer unprecedented potential to detect early oncogenic transformation.Through collaboration with leading international experts in fragmentomics, glycomics, and cancer screening, PRECISE-PANC will define the molecular landscape of pancreatic high-grade dysplasia. The ultimate aim is to shift screening for high-risk individuals from non-specific imaging to molecular detection, enabling curative surgery before invasive cancer develops.This proposal is timely, feasible, and based on unique existing infrastructure. It holds potential to revolutionize pancreatic cancer screening, reduce unnecessary surgery, and save lives.
